Associate Clinical Director of Nursing PracticeCenterLight Health SystemTiffany Bacchus is a licensed, board-certified family nurse practitioner with a total of 23 years in the nursing profession. She was born and raised in Flatbush, Brooklyn, New York. She attended Clara Barton High School, where she studied nursing, and at the age of 18, became a licensed practical nurse. She worked her way up the ladder and obtained her RN degree from Lehman College while working at the bedside on a trauma surgery unit in a city hospital.
After gaining her Bachelor of Science degree, she found a position at NYP Brooklyn Methodist Hospital, where she spent 13 years of her career. She continued to work at the bedside as a registered nurse on a bariatric surgery unit, but her quest for career advancement continued. She achieved her Master of Science in Nursing and became a board-certified practitioner. Her passion for healthcare equity and quality led her to ambulatory care, where she functioned in care coordination and later in quality reporting to NCQA. This role led to a position at NYP Brooklyn Methodist Hospital in administration, working as a patient quality specialist before returning to community primary care at a startup company in 2022. The time spent at the startup practice afforded her the ability to practice medicine in local underserved neighborhoods, where patients had multiple chronic conditions and poor access to optimal care. The practice model was that of wrapped care, where the patients had social, medical, and behavioral health services available to them in an effort to prevent condition decline and unnecessary hospitalizations. She worked in this role until a new position presented itself, working in a similar PACE program that served the geriatric population with a very similar care model. Today, she serves as an Associate Clinical Director for the New York Market at a national health plan. |
